Registration Conditions By sending in your signed application, you have agreed to the conditions and terms listed on this page. Please read carefully! Registration To register for any of the Can-Pro Programs, please complete the application and include the program fee as follows: 1. A cheque at the time of application for $379.00 CDN Or 2. A cheque at the time of application for $279.00 CDN and a post dated cheque for June 1, 2010 in the amount of $100.00 CDN Make all cheques or money orders payable to: Can-Pro Hockey Academy 127 Sackville Drive S.W. Calgary, Alberta T2W 0W4 Your cancelled cheque is your notice of enrollment. Conditions and Terms The hockey school, the arena, the people employed at the facility, its coaches, managers, and/or instructors are not responsible or liable for any injuries, lost, stolen, or broken articles while attending the program. We reserve the right to cancel or alter our programs as dictated by enrollment or any unforeseen circumstances which may lead to the cancellation of our programs. Refunds may be issued up to 60 days prior to the start of the program. Medical refunds will be given up to and including June 1, 2010. Cancellation for medical reasons must be accompanied by a doctors note. All requests for refunds will be subject to a 30% administration processing fee. All NSF cheques will be subject to a $25 surcharge. Under no circumstances will a player who is expelled or leaves of their own accord be given a refund. |
